Prognostic scoring systems in critically ill clinical and surgical patients: prospective cohort

Resumo

Objective: To compare prognostic scoring systems in critically ill, clinical and surgical patients admitted to the Intensive Care Unit (ICU). Method: This was a prospective cohort study with a follow-up of 141 patients. Information was collected to characterize the individuals, in addition to the Simplified Acute Physiology Score 3 (SAPS 3), Sequential Organ Failure Assessment (SOFA) and Nursing Activites Score (NAS). The associations between the variables were verified using the Chi-square test and Fisher's exact test, with values of p < 0.001 being significant. Results: More than half were female, 51.77%, with a median age of 60 (49 – 70) years. Mortality was higher among clinicians [12.50% vs 1.45%, p = 0.018] than among surgical patients. Those who died had higher severity SAPS 3 [62 (52 – 74) vs 41 (35 – 57), p = 0.021], SOFA [5.56 (2.85 – 8.50) vs 2.07 (0.36 – 3.90), p = 0.016] and required a higher NAS workload [98 (89 – 103) vs 94 (81 – 99), p = 0.207]. Conclusion: Patients of clinical origin have a higher severity according to the SAPS 3 and SOFA and higher mortality when compared to surgical patients, with greater nursing workload according to the NAS.
